Convert between .DSK and .BIN w/CP/M OS?

Hi,

I don't have a problem copying files between the two formats, but how do I add a CP/M 2.2 BOOT to a "BIGDISK.BIN" image?

Currently, Disk B and Disk C are blank ... A: contains the "ALTAIR/IMSAI CPU CARD. 61K CP/M 2.2"
1 - Disk A = DISK-A.BIN -EXISTS = 8" SSSD 250K contains the "ALTAIR/IMSAI CPU CARD. 61K CP/M 2.2" image
2 - Disk B = DISK-B.BIN -EXISTS = 3.5" DSDD 1.44 M Blank
3 - Disk C = DISK-C.BIN -EXISTS = 3.5" DSDD 1.44 M Blank
4 - Disk D = DISK-D.BIN -EXISTS = 3.5" DSDD 1.44 M Filled with all the files from the original A:, B:, C: and D: drives ... but not BOOTable.

Now I want to format the B: drive as a BOOTable "ALTAIR/IMSAI CPU CARD. 61K CP/M 2.2" so I can swap it into the A: drive position and add a new blank Disk B = DISK-B.BIN -EXISTS = 3.5" DSDD 1.44 M to have maximum storage space.
